The Genesis Probe

Successfully launched on a mission to gather solar particles in 2001, Genesis was the first probe to gather particles beyond the orbit of the Moon. While the scientific community was aflutter with the thought of studying new space dust, the probe's improperly packed parachute failed to open on re-entry, causing it to crash into the Utah desert. Subsequently, most of the samples were contaminated.
